[CentOS] kernel: Page has mapping still set

Have an HP Proliant DL380 that has started to oops with the following
messages:

serious situation. However if you

The "nv" module isn't used, so that's not the problem...

I've seen limited references to this problem, although some hint that it
may be a memory failure on the machine.  Has anyone else worked this
problem and can confirm or deny that?

Has happened now on kernels kernel-smp-2.4.21-37.0.1.EL and
kernel-smp-2.4.21-40.EL.
