What happens when you try the following?
iptables -t nat -A PREROUTING -I eth1 -d 69.54.130.16 -p tcp -m tcp
--dport 25 -j DNAT --to-destination 10.10.10.12
Can we get an output of your iptables?
"iptables -L -nv --line-numbers"

Here is the corresponding command from my iptables script:
EXTDEV=eth1
EXTIP=69.54.130.16
SMTPIP=10.10.10.12
$IPT -t nat -A PREROUTING -i $EXTDEV -p tcp -d $EXTIP --dport 25 \
-j DNAT --to-destination $SMTPIP
