I have a bad feeling about this, to quote Han Solo. lspci not showing it?
First thing I'd do is power cycle the box. If it shows up, fine. If not,
or if it shows up, then vanishes in the near future... is this an
on-the-m/b NIC? If so, start spec'ing out a new m/b or box.
