Amazing what brainstorms hit after you click "send". Unfortunately, the thoughts didn't help.
I tried again using the kernel parameters from the CentOS 4.8 grub kernel parameters that still work.
ide0=ata66 ide1=ata66 hdc=ide-cd lapic
AFAICT, same results.
