B) your systems USB ports are flakey
Will search for a BIOS image upgrade.

Plugged directly into the MB port.
I've run into C) quite a lot, where front panel USB ports don't work reliably at USB2.0 speeds when doing large and/or complex operations. I suspect the internal wiring is inadequate for the task.
