The install kernel CentOS 4 uses is different from the install kernel RHEL 4 uses. You'll need to rebuild the driver disk using modules compiled for the 5.0.3 kernel (IIRC).
Well, I give up using the x86_64 version, so I used the x86 version instead, and it works well. I guess the driver for x86_64 version is not stable enough.
