The driver seems to be for RedHat 4 U2 Which would coincide with Centos 4.1. You would need to find a disk of Centos 4.1 x86_64.
Your math does not resemble the math the rest of us use. RedHat 4 U2 equates to Centos 4.2. The numbers line up quite well.
