I don't see your question in the rest od the stuff you have quoted.
I don't really know what the HW problem is ...
You have a hardware failure. Fix that. After that you should be able to install (are you able to do a manual install on that machine?).
The weird thing is when i tried to install the machine again, everything was o.k. (but i have several machines that i need to install using PXE, and failure is problematic for me).
This is why i try to find a solution to the failure.
