I did not see any code about smtp connnections...
have you correct connection parameters for your smtp? smtp server ? smtp authentication(TLS. ldap. or static ip)? smtp port? with little googling look at the samples... and from web browser error console shows what ? and apache( or IIS or other ) logs ? Regards
