I think I can also do that (I've a fast mirror, http://archive.cs.uu.nl/mirror/Linux/centos/). When I started mirroring, CentOS didn't do DVD's on mirrors.
What should I change? (I now mirror eu-msync.centos.org::CentOS/ )

You need to be allowed to do that. You could also rsync the dvd isos from a different mirror close to you.

The apache version needs to be >= 2.2 or newer versions of lighttpd to do so normally.
That is one reason why we have two trees.

No, it was centos 4 x86_64. But I just upgraded the system to CentOS 5.1. So apache should be oke now (2.2.3).

In any case, I'm not going to switch over to a non centos.org machine to mirror from (i use my mirror to maintain university critical desktops and servers). So if there is a mirror stop currently, i forget about mirroring the DVD's.

It's definitely enough, once we have revamped our mirror setup. Your mirror is currently a "regular" Tier 1 mirror, without the ACL to reach the CentOS dvd rsync servers.
In the near future, I would like the reorganise the way it's done (with/without dvd.centos.org -> large Tier1 -> Tier2 -> ...) The current ACLs on the dvd servers was given on 1st arrived 1st served (more or less). Now we can't accept any other direct rsync on our servers. That's not good and that should be solved asap. But maybe not in the middle of the 5.2 release.

A common workaround that is often mentioned on Fedora's mirror-list is to redirect .iso downloads to ftp URLs. Even if your http server can support > 2GB files, some http clients cannot (old wget for example). Here is the Apache config we use:
RedirectMatch 301 /(.*.iso)$ ftp://<hostname>/$1 RedirectMatch 301 /(.*/iso/*)$ ftp://<hostname>/$1
(replace <hostname> with your FTP server's hostname)
