Is there a reason for your use of '0x82000000' instead of '0x4002000000'?
'0x82000000'  is by default kern_addr_r & kernel_addr_r and looks wrong 
to me.

I wouldn't have thought so. Perhaps your use of '0x82000000' is causing 
the problem.
