There was a simple mistake where something which should have been left
internal was not removed. It happened, there was an apology and we
move on. It isn't the first time this has happened somewhere and it
will definitely not be the last. Trying to 'fix' it so it never
happens again is how you end up with committees reading documents
before they are approved for disclosure and everything sounds like a
Queen's speech.
