I may be crazy, But using the 5.4 i386 netinstall pointing at the /centos/5.4/os/i386 I get a error about the directory tree not maching boot media same with /centos/5/os/i386 Does this with my up to date mirror, my upstream (linux.mirrors.es.net) and mirrors.kernel.org
