So, I identified the issue : the jquery/javascript used on
http://centos.org/download/mirrors treats the first line as the header
line, and your mirror is the first on that list , so it was "skipped"
by that javascript.

I've added now an extra line in that .csv file (so a "header" line)
and now your mirror shows up on http://centos.org/download/mirrors/
