[CentOS-pt-br] Erro init (kernal panic)

Boa tarde,

Possuo um server com Raid 1 e as partições criadas em LVM, e hoje ao
iniciar o mesmo ocorreu o seguinte erro:

Raid set " nvdia_eeddffhhf" was actived
Creating root device
Mouting root file system
kjournald starting comit internal 5 seconds
Ext3-fs  : mounted filesystem with ordered data mode
Setting up otrher filessustems
Setting upnew root fs
no fstab.sys, mounting internal defaults
Switching to new root and running init
unmounting old /dev
unmounting old /proc
unmounting old /sys
exec of init (/sbin/init)failed!!!: no such file or directory
kernal panic-not syncing: attempted to kill init!

dei um boot pelo cd do centos no modo rescue, e rodar um chroot em
/mnt/sysimage,ocorre um erro informando que o /bin/sh não foi encontrado.

alguém tem alguma dica ?
