But it's TX4000 (ata) driver ??!

Ok but have you ever tried to remove a hard disk, re-plug in it ?
Did you do stress tests, while disks are in reconstruction state ?
Anyway it's the TX4000 and it's an ATA controller so it has nothing to
do with SATA ones...
