the milter is for not going to jail if your server is for
customers und you silentry drop an important false positive
because your wrong setup don't reject it in a proper way

a smart MTA would use blacklists with weight and postscreen
(not with the CentOS6 base packages) to reject 90% of the
noise before the content filters and then use spamd as
milter to proper reject spam instead accept and drop
