The resolution to the dual HDMI problem was to upgrade to CentOS 7.5.

All CentOS versions from 7.5 to 8.1 work with no problems.

Hi John, why not try something newer than CentOS 6.8 ?
Maybe not the latest 8.1 but 7.7 is quite recent and stable.

Your (integrated) video card is :
