What is supposed to happen to a box running kernel-2.6.9-22.0.1.106.unsupportedsmp now that yum is offering to install the stock 2.6.9-22.0.2.EL version even with the centosplus repo enabled?

2 > 1, so yes, it should be offering to install it. Feel free to hold out until the unsupported kernel is ready.

This would happen IF you don't follow the advise that is here:
http://mirror.centos.org/centos/4/centosplus/Readme.txt
Now ... if you do follow the advise and block reading kernel from the [base] and [updates] repos then nothing happens :)
