AMD x86_64 support was provided with the CentOS 3.3 release. To my knowledge, there isn't an Itanium build. No idea if/when one is planned.

Depends on the CPU and the OS. The AMD x86_64 CentOS (and RHEL) build will support 32-bit apps.
