It should be created if needed. Does /var/log/maillog show mail being sent to root? Is there an alias that makes it go elsewhere?

Depends on which MTA he's using. postfix for example doesn't deliver to root. So setting up an alias "root: user" is probably the best idea anyway - keeps one from working as root too much.
