I am looking at using rpm-ostree and CentOS atomic. Is it possible to update rpm-ostree with a usb stick? As in provide a version as a download for updating offline devices.

I don't really know if this works, but I would try rsyncing the entire "atomic/x86_64/repo" directory from a nearby mirror to an USB stick, and then editing /etc/ostree/remotes.d/centos-atomic-host.conf to have url=file:///path/to/atomic/x86_64/repo depending on where you have your USB mounted.
The three slashes are intentional; the third one signifies the root directory /
